The RN will conduct supervisory home visits for pediatric patients and manage clinical documentation, including CMS-485 plans of care. They will also collaborate with physicians and caregivers to ensure high-quality continuity of care.
CareSense is seeking a Pediatric Registered Nurse (RN) to support our growing home care program. This is a hybrid position, with time split between the office (documentation, coordination, and case management) and the field (supervisory home visits). The RN will play a key role in ensuring our pediatric patients receive safe, high-quality care in their homes.
Key Responsibilities
Travel to pediatric patients’ homes to complete RN supervisory visits in accordance with payer and agency requirements.
Complete and update CMS-485/Plans of Care and other required clinical documentation for pediatric home care cases.
Perform focused nursing assessments and document skilled needs, progress toward goals, and any changes in condition.
Review charts in the office to ensure documentation is complete, accurate, and compliant with regulations and payer standards.
Collaborate with coordinators, administrators, and the clinical team to resolve issues, support staffing, and ensure continuity of care.
Communicate with physicians, case managers, and other providers regarding patient status, care needs, and plan-of-care changes.
Provide education and support to caregivers and families on the child’s care needs, equipment, and safety.
Offer clinical guidance to direct care workers/home care aides working with pediatric clients.
