403b retirement plan with company match after one year Complimentary access to VNA Mobile Health Clinic Free CEU credits Immediate Access to Employee Assistance Plan (EAP) Several local and national discounts The Visiting Nurse Association of the Treasure Coast is seeking a Per Diem RN to assume responsibility for providing skilled nursing care and health teaching to patients and their significant others in the home setting.

Responsibilities

Administers medications and treatments as prescribed by the physician. Observes patient condition to monitor response to treatment. Counsels and teaches patient and significant others to accomplish goals. Conducts physical assessment and history to determine health problems and determines intervention required. Establishes and follows a care plan which addresses problems requiring intervention. Assesses patient and family for appropriateness of and acceptance for home care. Serves on agency committees as requested. Assists in orientation of new staff nurses. Provides supervision to Home Health Aides and prepares their assignments. Participates in community health projects. Maintains clinical records according to agency, State and Federal guidelines. Participates in QA activities as assigned. Understands medico-legal implications of records and recordings. Determines the focus of each patient visit. Refers to other resources within the agency and to appropriate other community agencies as needed. Recognizes families' abilities, limitations, and stresses, and helps them cope with their changing roles. Coordinates services of interdisciplinary team and participates in case conferences. Formulates appropriate discharge plans. Reports changes in patient condition to primary nurse and MD. May participate in agency in-service education programs. Attends all staff meetings requested by supervisor. Adheres to established productivity standards for the Per Diem RN position. Minimum productivity is 30 points per month. These points are to be completed during the standard work week (Monday – Friday). This is the equivalent of 6 full workdays per month. Per diem associate must commit to working full workdays; partial day availability will not be permitted. The associate is expected to be proactive in achieving minimum productivity standards as a requirement for continuing in their per diem role. In event the clinician has no control over circumstances that may negatively impact their productivity point accrual; they will not be held responsible for completion of fewer points than required in a month. Must work a minimum of one weekend a month. Required to work one summer holiday and one winter holiday.

Qualifications

Must be currently licensed as an RN in the state of Florida. Must have at least one year of recent Home Health experience. Must have working knowledge of Homecare Homebase EMR, and Oasis documentation Case Management Knowledge is a plus *This position requires a screening through the Care Provider Background Screening Clearinghouse. Requirements

Must be a licensed RN in Florida with at least one year of recent home health experience. Proficiency in Homecare Homebase EMR and Oasis documentation is required.
