Overview We are seeking a compassionate Home Health Aide (HHA) to provide care and support to hospice patients throughout Tompkins and Cortland Counties. Follow the patient's plan of care as directed by the nurse. Report changes in patient condition to the appropriate nurse. Document care provided accurately and timely. Assist patients with prescribed exercises. Provide personal care, bathing, and hygiene assistance. Prepare meals and assist with feeding when needed. Perform light housekeeping to maintain a safe environment. Make beds and help maintain patient comfort. Provide respite support for family caregivers. Attend required in-services and team meetings. Maintain a safe environment and report concerns promptly. Perform other duties as assigned.
Current New York Home Health Aide (HHA) Certification required. Valid driver's license. Reliable transportation. Ability to travel throughout Tompkins and Cortland Counties.
