About the role

The In-Home Medical Day Care Provider will provide personal care and supervision for individuals with special health care needs in their home. Responsibilities include implementing care plans, tracking personal belongings, and communicating with families and staff.

ChildServe is seeking an In-Home Medical Day Care Provider to provide personal care and supervision for an individual with special health care needs in their home.

Schedule Various hours between 9am – 5pm M-F

  • How You’ll Make an Impact
  • Provide direct care and assist individuals with daily living and behavioral needs in their home
  • Implement care plans and provide ongoing feedback regarding additional or changing care plan needs
  • Perform duties related to the tracking and upkeep of participant’s personal belongings
  • Responsible for timely and accurate communication regarding services with families, staff, and supervisors
  • Transport individuals to outings in the community

What You’ll Need

  • Must be at least 18 years old to apply
  • Experience working with or knowledge of individuals with special health care needs preferred
  • Provide your own transportation
  • Must be willing/able to become an approved driver for ChildServe, if required
  • Ability to electronically document services provided
  • Ability to communicate effectively in reading, writing, and speaking the English language
  • Ability to lift up to 35 lbs. be mobile within the assigned work area, and frequently bend, lift, reach, and stand
