Provide palliative care through health history evaluations, medical diagnosis, and the development of individualized care plans. Perform face-to-face assessments and collaborate with an interdisciplinary team to prescribe treatments and medications.

What You’ll Do

Provide palliative care through evaluation and assessment of health history, diagnosing medical conditions.

Develop individualized plans of care.

Complete face to face assessments and prescribe medications or treatments.

Collaborate with the interdisciplinary team.
