Provide expert, hands-on end-of-life care to patients while following state nursing rules and established care plans. Coordinate with the hospice interdisciplinary team to manage symptoms and support patients and their families.

Graduate of an accredited practical nurse or vocational nursing program. Three years nursing experience. Community health/hospice or medical/surgical experience is preferred. Currently licensed as an LPN/LVN in the State. Comply with accepted professional standards and practice. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Possess and maintain current CPR certification. Understand philosophy of hospice concept. Also understand needs of the terminally ill. Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems. Demonstrate good verbal and written communication, and organization skills.
