Collects data from Inquiry Calls, assessments, etc. and inputs into Health Management System in a timely manner.
College degree in Nursing or Social Work.
Candidates must have a college degree in Nursing or Social Work and a current Ohio license as an RN or Social Worker. Two to three years of related experience and certifications in CPI, CRP, and TAPS are preferred or required.
