About the role

Provide care coordination and targeted outreach for HUSKY Health members in rural areas to improve healthcare access and outcomes. Perform assessments, medication reconciliation, and collaborate with multidisciplinary teams to implement care plans.

Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking an LPN Care Coordinator for the Rural Health Transformation (RHT) Program.

The Rural Health Transformation (RHT) Program is a federal grant that empowers states to strengthen rural communities across America by improving healthcare access, quality, and outcomes by transforming the healthcare delivery ecosystem.

Primary Responsibilities

  • Under the direction of the Care Management (CM) Manager of Rural Health Transformation, provide care coordination support for eligible HUSKY Health members residing in rural areas.
  • Conduct targeted outreach to rural members to offer engagement in the CHNCT Intensive Care Management (ICM) program.
  • Complete designated assessment scripts, perform medication reconciliation, provide condition and medication related coaching, support implementation of the care plan, and assist with care coordination activities to facilitate access to HUSKY Health services and community resources.
  • Collaborate with the multidisciplinary care team to address barriers to care and support achievement of care plan goals.

Tasks Performed

Works in collaboration with RN care managers and team leads to coordinate services, address member needs, and support care plan goals.

Completes risk screenings/assessment scripts for identification of member needs and provides specified interventions to address the immediate needs of members.

Refers members to an RN care manager when high-risk medical, behavioral, or social issues are identified during the call.

Responds to telephonic inquiries from members.

Coaches and assists members in achieving successful self-management skills.

Provides appropriate written/digital educational resources to support the member's learning.

Participates in interdisciplinary case rounds/collaborative meetings as appropriate with other members of the Rural Health Transformation team.

Completes documentation in the electronic care management documentation system.

Performs other duties as assigned.

Essential Functions

Telephonic/videoconference member outreach, structured assessments, and coaching.

Participates in virtual member learning groups and organized community activities upon request.

Maintains knowledge of covered benefits for CT Medicaid, HUSKY Health programs.

Desired Education: Two years post-secondary schooling

Desired Degree: Associates degree preferred

Desired Major: Vocational nursing; required

Desired Job Experience: 1 to 5 years nursing experience; required

Other Qualifications

Excellent organizational skills, ability to manage multiple priorities concurrently. Strong oral and written communication skills. Strong computer and keyboard skills. Team player. Ability to exercise independent and sound judgment in decision making. Managed care experience preferred. Knowledge of HUSKY Health contract requirements preferred. LPN license must be current and unrestricted. Current and unrestricted motor vehicle operator's license and reliable transportation.

Certifications Required: Active Licensed Practical Nurse License

Requirements

Requires a current and unrestricted LPN license and 1 to 5 years of nursing experience. An associate degree in vocational nursing is preferred, along with a valid driver's license and reliable transportation.
