We’re seeking a compassionate Registered Dietitian to join our interdisciplinary hospice team, supporting patients in our Hospice House and community-based settings. This is a meaningful opportunity to practice truly patient-centered care. What You’ll Do Provide nutritional assessments, education, and consultation for hospice and home care patients Conduct home visits and collaborate with nurses, social workers, and clinicians Support kitchen oversight in our Hospice House, ensuring NYS food safety compliance Educate staff and volunteers on therapeutic diets and safe food handling Complete documentation within 24 hours Participate in IDT and care management meetings
