The Occupational Therapist (Per Diem/PRN) is responsible for providing high-quality, patient-centered therapy services in the home setting. The OT evaluates, plans, and implements individualized treatment programs to improve patients’ ability to perform daily activities, promote safety, and enhance overall well-being. This position works collaboratively with the interdisciplinary home health team to ensure effective, coordinated care.
Key Responsibilities Conduct comprehensive assessments to evaluate functional abilities, home environment, and rehabilitation potential.
Develop individualized care plans and treatment goals that address physical, cognitive, and psychosocial needs.
Provide therapeutic interventions to improve strength, coordination, fine motor skills, and independence in activities of daily living (ADLs).
Educate patients and caregivers on adaptive techniques, safety strategies, energy conservation, and use of assistive devices or equipment.
Recommend and coordinate durable medical equipment (DME) and environmental modifications as needed.
Document all evaluations, treatments, and progress notes in compliance with Wellpointe Home Health policies, state, and federal regulations.
Collaborate with nurses, physical therapists, speech therapists, and medical social workers to ensure a cohesive plan of care.
Participate in case conferences, team meetings, and quality improvement initiatives as requested.
Communicate effectively with physicians and care coordinators regarding patient progress and needs.
Education: Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
Current and valid Occupational Therapy license in the State of California.
Minimum of one (1) year of occupational therapy experience preferred; home health experience strongly desired.
Current CPR certification.

Requirements: Valid driver’s license and reliable transportation. Proof of automobile insurance. Strong organizational and time-management skills for per diem scheduling.
