Summary

  • RN case manager (CM) is responsible for planning, implementing, and coordinating health care resources/services to meet the physical and mental health needs of women at TNDR Chattanooga Re-Entry Center, preparing them for successful reentry into the community.

  • Duties include the following: Initial admission assessment and medication reconciliation Manage resident medication processes including organization, storage, receipt, and prescriptions Provide oversight of the daily dispensing of medication. Coordinate medication prescriptions and refills, communicate with medical providers, TNDR residents, and TNDR staff. Collaborate and connect residents with community health care providers and pharmacies. Participate in weekly treatment team meetings. Ensure timely and professional documentation per TNDR guidelines. Provide nursing triage to residents with medical or mental health concerns. Administer basic first aid to residents if indicated. Provide health education to residents. Provide consultation to TNDR staff regarding resident’s health conditions and/or medications. Additional responsibilities as assigned.

  • Experience: Bachelor’s of Science or Associate’s degree in Registered Nursing Minimum of 1 year experience as a Registered Nurse Valid unrestricted Tennessee Registered Nursing license Knowledge of psychiatric disorders and alcohol and substance abuse Knowledge of community resources and health care service delivery across a continuum of care

Requirements

Candidates must hold a Bachelor's or Associate's degree in Registered Nursing and a valid unrestricted Tennessee RN license. A minimum of one year of experience as an RN and knowledge of psychiatric disorders and substance abuse is required.
