Perform comprehensive health assessments and develop individualized care plans Diagnose and manage acute and chronic conditions, including: Diabetes Hypertension Hyperlipidemia (cholesterol management) Prescribe medications and manage ongoing treatment protocols Conduct STI/STD screening and treatment Perform physical and wellness exams (men and women) Provide patient and family education on disease prevention and health maintenance Participate in weight loss and lifestyle management programs Support home-based primary care services for eligible patients Maintain accurate and timely electronic medical records (EMR) documentation Collaborate with physicians and interdisciplinary healthcare teams Participate in quality improvement and patient safety initiatives Engage in community health fairs and wellness education programs Support nursing and clinical training/education initiatives
Active Nurse Practitioner license in the state of Georgia Master’s or Doctorate degree in Nursing (MSN or DNP) Minimum of 2 years clinical experience in primary care, urgent care, or related setting Current certification in BLS (required) and ACLS (preferred) Medicare and Medicaid credentialing required (or ability to obtain upon hire) Proficiency in electronic medical records (EMR) systems Strong clinical judgment, communication, and interpersonal skills Ability to work independently and collaboratively in a fast-paced environment Commitment to patient-centered care and continuous professional development Preferred Experience
