Full-Time position available for a Pathways Community Health Workerfor our Care Management team in Vancouver, WA.
This position worksclosely with all clients, community organizations, and otherdepartments.
This culturally and geographically connected individualserves as a link between underserved communities and existingcommunity resources.
Candidates with a background in communityoutreach or case management are preferred.
Knowledge of communityresources is preferred.
Through home visiting, this individual assistsclients in overcoming barriers to health, social services, educationand employment.
Candidates who are bilingual in Russian or Spanish arehighly preferred.Driving to meet clients in homes, clinics, and hospitals is required.This person must have a valid driver license, proof of auto insuranceand a vehicle safe for daily use.
Prior to hire this person mustsubmit a driver's abstract demonstrating that they are safe to driveas deemed by Sea Mar's Health and Safety/Compliance Officer.
Withinthe course of employment, a current driver's abstract may berequested.
