The Outreach Coordinator will facilitate care coordination to patients and patients' families enrolled in care management services and assist with data management and reporting for care management services. The Outreach Coordinator will also assist in coordinating referrals to self-management education and support services and provide outreach for population health management.

Bachelor’s Degree in a health related field preferred. Medical Assistant or Licensed Practical Nurse license preferred.
