Oak Medical is seeking an experienced and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing clinical team and help establish an innovative Telehealth Triage Program serving residents in Skilled Nursing Facilities and Assisted Living Facilities.
This unique position combines chronic disease management, routine follow-up care, and same-day urgent clinical triage delivered through telehealth technology. As a key member of our care team, you will collaborate with facility staff, primary care providers, and interdisciplinary teams to improve patient outcomes, reduce unnecessary hospitalizations, and provide timely access to high-quality medical care.
This position is based at our corporate headquarters in Waukesha, WI, where it works in close partnership with our leadership team, all of whom are also based in the corporate office. The role depends on both structured collaboration through scheduled meetings and the day-to-day communication, problem-solving, and relationship-building that naturally occur when team members work together in person.
