Oak Medical SC is seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to provide acute and chronic care management for adult and geriatric patients throughout our Madison Area, including Beloit, Watertown, Lake Mills, Mt. Horeb, Lodi, DeForest.
While covering colleagues on PTO, you'll collaborate with physicians, specialists, and facility care teams to develop and implement comprehensive treatment plans that improve patient outcomes and quality of life.

What You'll Do
Provide primary care services for adult and geriatric patients with acute and chronic medical conditions.
Evaluate patients, establish diagnoses, prescribe medications, and perform diagnostic and therapeutic procedures within scope of practice.
Develop individualized treatment plans in collaboration with physicians and interdisciplinary care teams.
Coordinate care with specialty providers, including wound care, psychiatry, and other consulting services.
Communicate changes in patient condition and treatment plans with patients, families, facility staff, and healthcare team members.
Maintain timely, accurate, and compliant documentation.
Build strong relationships with patients, families, and facility staff to support exceptional care.
Perform other duties as assigned.
