About the role

The LPN will assist RNs with patient assessments, implement rehabilitation nursing interventions, and deliver care based on interdisciplinary plans. They are responsible for performing quality improvement assessments and ensuring patient safety through effective pain management and education.

Schedule: 3x12, Days (7a-730p), Weekends and Holidays Rotate

How you’ll contribute

A Rehab LPN/LVN who excels in this role

Assist the RN with initial assessments and patient reassessments per policy.

Identify and implement appropriate rehabilitation nursing interventions.

Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.

Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.

Assess and reassess pain; use appropriate pain management techniques and educate patients and families.

Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.

What we are looking for

Applicants should have a current Ohio LPN or LVN license and graduate of an accredited Licensed Practical/Vocational Nurse program. Additional requirements include:

Certifications: BLS required; IV certification with 6 months of hire required.

Experience: 6+ months LPN/LVN in Medical/Surgical preferred

Requirements

Applicants must hold a current Ohio LPN or LVN license and have graduated from an accredited nursing program. Additional requirements include BLS certification, the ability to obtain IV certification within 6 months, and preferred experience in a medical/surgical setting.
