Position Overview We are seeking a compassionate and motivated Nurse Practitioner to join our healthcare team in Charleston, South Carolina. This position combines in-clinic patient care with remote responsibilities, allowing providers to deliver comprehensive care across multiple settings. The Nurse Practitioner will collaborate closely with physicians and clinical staff to evaluate patients, monitor treatment progress, support care coordination, and ensure an exceptional patient experience.
Conduct patient evaluations, assessments, and follow-up visits in both clinic and telehealth settings Review patient history, symptoms, diagnostic findings, and treatment progress Assist in developing and implementing treatment plans under physician supervision as applicable Document patient encounters accurately and thoroughly within the EMR system Collaborate with physicians, therapists, and clinical staff regarding patient care and coordination Monitor patient progress and communicate recommendations as needed Provide patient education regarding diagnoses, treatment plans, and recovery expectations Assist with in-clinic procedures, therapies, and treatments as appropriate Ensure compliance with clinic protocols, healthcare regulations, and safety standards Support daily clinic workflow and operational efficiency
Active Nurse Practitioner license in South Carolina Strong clinical assessment and patient care skills Excellent communication and interpersonal abilities Ability to work independently and effectively manage responsibilities in both remote and clinical environments Comfortable utilizing telehealth platforms and electronic medical record systems Experience in neurology, rehabilitation, pain management, concussion care, or TBI-related care preferred CPR/BLS certification required
