Position Name: Physical Therapist (PT) Home Health Location: Lacey, Washington Specialty: Home Health Start Date: ASAP Employment Type: Full-time, Travel, Contract, Temporary Contract Length: 13 Weeks Pay Rate: $2600 to $27000 Weekly Gross pay based on a 40-hr work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers) Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Home Health Shifts available: Days Shift timings: 8 Hours; Days, 8:00 am - 5:00 pm
Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri
Provide Physical Therapy services for Home Health Patients Consult with the physician and an interdisciplinary team in the development of a plan of treatment Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential Plans and prepares treatment plan based on evaluation of patient data Provide in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventing disability Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home Drive to patient homes in the assigned territories on a daily basis. Complete charting and patient documentation in timely manner as per client policies and procedures Territories: Thurston, Lewis, Mason, Pierce counties (Need would be in any of the territories)
