Position Name: Hospice Registered Nurse Location: Lexington, Ohio Specialty: Home Health Start Date: ASAP Employment Type: Full-time, Travel, Contract, Temporary Contract Length: 13 week contract Pay Rate: Range $2120 to $2200 Weekly Gross pay based on a 40-hr work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers) Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Home Health Shift: 8 Hours; Days, 8.00 am - 5.00 pm Schedule: 40 hours/week guaranteed, 5 x 8-hr shifts per week, Mon-Fri; Weekend Rotation, Rare on-call Position Requirements: Registered Nurse: 2 years - Required Recent/Current Hospice Nursing experience in a home setting: 1 year within the last 3 years – Required Home Care Home Base (HCHB) charting experience – Required Timely communication and documentation - Required Must have valid Driver’s License and Personal Auto Insurance Must be Covid Vaccinated or provide medical or religious exemption Education, License and Certification Requirements: ASN or an equivalent – Required BSN or an equivalent – Highly preferred RN License for State of Ohio or Compact - Required BLS Certification from AHA or American Red Cross - Required
Provide and coordinate care for assigned hospice patients in comfort of their homes. Develop and implement nursing care plans for all type of assigned visits. Support patient comfort and dignity Empower patients and families to reach healthcare goals by educating them on pain management. Provide Home health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures. Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure. Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented. Notify physicians of significant changes in status. Must communicate with the office promptly. Must document and complete charting for all patient visits in a timely manner in accordance with client policies and procedures. Territories to cover: Mansfield, OH, and surrounding cities
Requires a Registered Nurse license in Ohio or Compact with at least 2 years of experience and 1 year of recent hospice home care experience. Must be proficient in HCHB charting and possess a valid driver's license and BLS certification.
