Position Name: LPN Home Health Location: Zanesville, Ohio Specialty: Home Health Start Date: ASAP Employment Type: Full-time, Travel, Contract, Temporary Contract Length: 13 weeks contract Pay Rate: $1680 Weekly Gross pay based on a 40 hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers) Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments
Home Health Shifts available: Days Shift timings: 8 Hours; Days, 8.00a - 5.00p
40 hours / week guaranteed, 5 shifts per week, Mon-Fri, No weekends or On-call | 30 Weekly Productivity expectation
Provide and coordinate care for assigned Home Health patients under supervision of RN Develop and implement nursing care plans for patients as assigned. Empower patients and families to reach healthcare goals by educating them on disease management. Provide Home Health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures. Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure. Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented. Notify physicians of significant changes in status. Document and complete charting Territory: Coshocton and Muskingam counties
