Sage Group HealthCare Staffing is seeking an RN Home Health for a travel/contract position in Green Bay, WI. We are accepting both local and traveler candidates.
Unit: Home Health Shifts available: Days Shift timings: 8 Hours; Days, 8a – 5p with some on-call
Provide and coordinate care for assigned patients in comfort of their homes. Develop and implement nursing care plans including Start of Care and admissions as assigned. Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy. Empower patients and families to reach healthcare goals by educating them on disease management. Provide Home health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures. Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure. Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented. Notify physicians of significant changes in status. Attend IDT meeting as required Complete patient visits as assigned to meet the minimum required weekly productivity points (30) Document and complete charting for all patient visits within 24 hours. Territories to cover: Green Bay and outlying areas
Requires a Registered Nurse license with at least 2 years of experience, including 1 year of Home Health and proficiency in OASIS and HCHB charting. Must possess a valid driver's license, auto insurance, and BLS certification.
