Sage Group HealthCare Staffing is seeking a Physical Therapist for a travel contract position in Colebrook, NH. We are accepting both local and travel candidates.
Hospital based outpatient clinic Shifts available: Days Shift timings: 8 hours, Days, 8.00 am to 4.30 pm
Provides all aspects of PT Services in a hospital based outpatient clinic and occasionally inpatient. The physical therapist will accurately assess, differentially diagnosis, and identify deficits in a patient’s ability to move and perform functional activities in their daily lives. Develop individualized treatment plans and performs evaluations. Establishes an individualized plan of care and selects treatment techniques that addresses the specific impairments identified in the evaluation and in accordance with physician orders. Conducts ongoing reassessment of the patient and their response to treatment. Completes legible, accurate and clear documentation within the established timeframe Assesses educational needs of the patient, family, caregiver, or significant other and provides education to meet those needs. Functions within scope of practice as defined by the state’s practice act. Performs other related duties as required and directed.
Requires at least 2 years of experience in outpatient physical therapy and a valid state PT license. BLS certification and prior inpatient or travel experience are preferred.
