Position Name: Registered Nurse Home Health Location: Albuquerque, New Mexico Specialty: Home Health Start Date: ASAP Employment Type: Full-time, Travel, Contract, Temporary Contract Length: 13 weeks Pay Rate: Range $2080 to $2200 Weekly Gross Pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers) Mileage Reimbursement: $0.54/mile for distance covered between patient home appointments

Provide and coordinate care for assigned patients in comfort of their homes. Develop and implement nursing care plans including Start of Care and admissions as assigned. Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy Empower patients and families to reach healthcare goals by educating them on disease management Case management and provide clinical oversight to interdisciplinary Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures. Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure. Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented. Notify physicians of significant changes in status Document and complete charting for all patient visits within 24 hours. Territories to cover: Greater Albuquerque area
Requires a Registered Nurse license with at least 2 years of experience, including 1 year of recent Home Health experience. Must be proficient in OASIS and HCHB charting and possess a valid driver's license with a reliable vehicle.
