Perform and document comprehensive patient evaluations and develop individualized treatment plans.
Assess patient conditions and determine physical therapy diagnoses prior to intervention.
Design and implement treatment programs to improve mobility, reduce pain, restore function, and prevent disability.
Monitor patient progress and modify treatment plans to maximize outcomes.
Deliver care in accordance with infection control and universal precaution standards.
Collaborate effectively with patients, families, physicians, and interdisciplinary care teams.
Ensure timely, accurate documentation and compliance with company standards.

Must be a graduate of an accredited Physical Therapy program with a current state license and CPR certification. A valid driver's license, auto insurance, and preferred experience in home health or with HomeCare HomeBase are required.
