Our Therapists are responsible for managing a caseload of patients requiring home-based therapy, primarily post-acute or subacute. Enjoy starting and ending your day from home!
Promote/provide physical therapy and/or occupational therapy services in the home to assist the patient in achieving his/her optimal level of functioning.
Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.
Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es)

Coordinate an interdisciplinary plan of care with nursing/physical/occupational/speech therapists, medical social workers, aides and dieticians.

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Requires a current NYS PT or OT license and registration along with a valid driver's license and reliable vehicle. A minimum of one year of experience as a PT or OT is required, with homecare or inpatient experience preferred.
