Provide exceptional preventive and chronic disease primary care for adults aged 65 and older. Collaborate with an integrated care team including NPs, PAs, and social workers to improve senior health outcomes.

We welcome physicians with Value-Based Care (VBC) experience as well as those eager to learn.
Board Certified in Internal Medicine or Family Medicine Clear and active medical license DEA Geriatric experience preferred HEDIS experience a plus Value-Based Care experience preferred–but not required; training provided NO RESIDENTS, NO NEW GRADS
