Provide exceptional preventive primary care for adults aged 65 and older within a value-based care model. Manage a daily census of 12-15 patients, focusing on chronic disease management and senior health outcomes.

We welcome physicians with Value-Based Care (VBC) experience as well as those eager to learn.
Board Certified in Internal Medicine or Family Medicine Clear and active medical license DEA Geriatric experience preferred HEDIS experience a plus Value-Based Care experience preferred–but not required; training provided NO RESIDENTS, NO NEW GRADS
