As a Physical Therapist in a Skilled Nursing Facility, you will help patients regain mobility and independence following illness, injury, or surgery.
Evaluate patients and develop individualized treatment plans Provide evidence-based physical therapy interventions Collaborate with physicians, nurses, and rehab team members Supervise Physical Therapy Assistants (PTAs) Document patient progress accurately and timely Participate in care planning and interdisciplinary meetings Educate patients and caregivers on mobility and safety Support discharge planning and functional outcomes
