About the role

The therapist is responsible for conducting psychosocial assessments and providing individual, group, and family therapies to adult patients. They must also develop individualized discharge plans and coordinate community resources for patient aftercare.

  • Location: Branches Monroe in Monroe, LA Brentwood Hospital is looking for a dynamic Counselor / Social Worker to join our team of professionals who are dedicated to providing help and hope to individuals and families for the achievement of improving their mental health.

  • Conducts psychosocial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient and their family members.
  • Provides group and family process-oriented therapies as well as conducts educational and other didactic groups for patients and family members using various professional treatment modalities.
  • Daily progress notes summarized patient’s progress toward treatment goals; Contact with family, significant others, community agencies recorded in progress notes.
  • Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patient’s aftercare needs.
  • Identifies and assesses family or community resources such as, group homes, or mental health practitioners to meet patients’ aftercare needs.

Qualifications

Education / Experience

Master's Degree in social work, counseling or related field. Experience in inpatient or outpatient mental health setting preferred.

Requirements

Candidates must hold a Master's degree in social work, counseling, or a related field and possess a valid LPC or LCSW license. Experience in an inpatient or outpatient mental health setting is preferred.
