The Social Worker coordinates services and manages the psycho-social needs of residents in a skilled nursing facility. Key duties include completing admission/discharge paperwork, creating care plans, and conducting MDS assessments.
Complete admission and discharge paperwork for residents.
Monitor the psycho-social and emotional needs of residents.
Create care plans and make amendments as needed.
Coordinate all parties in resident care conferences/special meetings with a multi-disciplinary team.
Complete assigned MDS interviews and MDS social services assessments.
Assist residents and families with room changes.
Oversee Medicaid- and Medicare-related paperwork and recordkeeping.
Respond to resident/family/staff concerns using confidentiality and professional judgment.
Required Qualifications:Degree in Social Work or related Current MN Licensure in Social Work or ability to successfully obtain license within 60 days. Proficiency in Microsoft Office and digital literacy for charting and interfaces with outside services. Ability to pass NetStudy 2.0 background screening.
