About the role

The Harm Reduction Worker manages daily operations of Drop-In Services, providing non-judgmental support such as laundry, showers, and food coordination. They are responsible for patient intake, facilitating support groups, and documenting all interactions in the electronic health record.

The Harm Reduction Worker is responsible for helping with the daily operations of Evergreen’s Drop-In Services, including the provision of services in a non-judgmental manner, such as coordinating showers, laundry, food and beverage, and individual and group support. This role ensures that services are provided smoothly, in a safe environment, and are tracked appropriately

As part of the essential functions of this role, the Harm Reduction Worker

Provides direct patient services in the Drop-In Center

Takes initiative on warmly greeting patients, patient intake, hospitality services, coordinating shower and laundry services, and creating, implementing, and facilitating groups

Documents patient interactions in the electronic health record (EHR) and actively collaborates with other providers and departments within EHS

Adheres to company policy regarding documentation standards and completion timeframes

Assists patients in identifying personal goals and follows up regularly to assess readiness to address their stated goals

Qualified candidate will have an Associate degree in public health or related field, or two years of experience working with at-risk populations; ability to travel between office locations as needed; have and maintain a current and valid NYS driver’s license, verified thought the NYS LENS system; excellent verbal and written communication skills, with bilingual English & Spanish preferred; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential

Associate degree in public health or related field, or two (2) years of experience working with at-risk populations; bilingual English & Spanish preferred

Required License: current and valid NYS driver’s license, verified through the NYS LENS system
