About the role

The Linkage Specialist engages individuals using substances to provide education on safer options and conducts harm reduction sessions including wound care and testing. They are responsible for linking patients to appropriate treatment options, specifically for Hepatitis C, and coordinating higher levels of care.

The Linkage Specialist is responsible for engaging individuals who use substances in discussion about their use and education on safer options. The Linkage Specialist targets participants of the Syringe Services Program and Drop-In Services.

As part of the essential functions of this role, the Linkage Specialist

Conducts individual harm reduction sessions with individuals using substances to provide treatment options, safer injection counseling, wound care, testing, and goal planning related to Harm Reduction Billing services

Available to conduct the above sessions with patients by appointment or on a walk-in basis for the Center of Behavioral Health as well as other departments of Evergreen Health

Fills in as necessary to conduct rapid and confirmatory HIV/Hepatitis C testing and enters affiliated documentation into AIRS, Medent and Testing Tracker Form

Links individuals with treatment according to test results, with an emphasis on Hepatitis C due to its prevalence among PWUD; this may include reminder calls about Hepatitis C treatment appointments and check-ins with patients and providers

Provides internal and external linkages for clients seeking a higher level of care

  • Qualified Candidate will have an Associate degree in public health or related field or two years of qualifying experience*; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential
  • “Qualifying Experience” means verifiable full- or part-time experience in care coordination/case management with the following populations: person with a chronic illness, and/or persons with a history of mental illness, homelessness, or chemical dependence*

This position is posted with a salary range. The actual rate of pay offered will be based on a variety of individualized factors, including but not limited to a candidate’s skills, relevant experience, educational background, internal equity, and market considerations.
