The Care Coordinator manages intake, enrollment, and individualized service planning for eligible participants. They provide non-clinical mentoring and coordinate resources with external providers to ensure quality care and compliance.

Education

Required - High school diploma or equivalent. Preferred - Bachelor’s degree in a related field.

Experience

Required - With a high school diploma, at least 3 years of professional work experience in a related field. Preferred - With a Bachelor’s degree, at least 1 year of professional work experience in a related field.

Job Summary

The Care Coordinator is responsible for using sound professional judgment and best practices to ensure quality services are provided to the service population while maintaining compliance with established program standards.

Essential Functions

1. Conduct outreach, intake, enrollment, and orientation activities for eligible participants. 2. Establish and maintain a trusting and professional working relationship with participants by providing regular contact. 3. Complete comprehensive participant assessments to identify strengths, needs, barriers, and goals. 4. Develop and maintain individualized service plan in collaboration with participant. 5. Provide case management and non-clinical mentoring services to participants. 6. Provide service planning and coordination, resource and referrals, basic needs support, and/or other support services to families on caseload. 7. Conduct systematic follow-ups for all families.

Measurable Deliverables

  • 1. Participate in community outreach activities, resource fairs, and partnership meetings. 2. Maintain accurate documentation in Salesforce and PEIRS within specified timeframes. 3. Coordinate services with external providers to ensure continuity of care and reduce duplication of services. 4. Maintain regular contact with participants through in-person, virtual, telephone, and community-based interactions. 5. Record all group and individualized services and activities in a complete, concise, and timely manner utilizing program forms into PEIRS (DFPS) and Salesforce (BCFS) software. 6. Meet with supervisor on a bi-weekly basis to evaluate status of caseload. 7. Attend all required annual trainings. 8. Travel is required within the service area. 9. Meet all deliverables as outlined in the contract. 10.
  • Demonstrate the ability to: a. Respond sensitively and competently to the service population’s cultural and socio-economic characteristics. b. Communicate effectively in writing and verbally in English. c. Work in a fast-paced environment. d. Maintain computer literacy required to meet the responsibilities of the position. e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team. f. Travel as needed. 11. Other job duties as assigned per grant requirements.

Requirements

1. Maintain certificates required for the position including but not limited to First Aid and CPR. 2. Valid Texas Driver’s license with a clear driving record and reliable transportation. 3. Pass a pre-employment drug screen and random drug screens. 4. Provide proof or work eligibility status upon request. 5. Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
