The Case Manager provides comprehensive services including intake assessments, case plan development, and home visits for clients. They are responsible for monitoring progress, facilitating health education, and maintaining compliance with state and federal standards.

Required – High School diploma or equivalent. Preferred – Bachelor's degree in related field.
Required – With a high school diploma or equivalent, 2 years of related experience and/or training in a related field.
The Case Manager is responsible for providing comprehensive case management services to clients, including but not limited to intake assessments, benefit assessments, goal setting, case plan development, home visits, progress monitoring via standardized testing tools, client advocacy, and referrals while maintaining compliance with established state and federal standards and requirements.
1. Conduct home visits or virtual visits to assess client's needs for case plan development, establishing rapport, and obtaining standardized assessments. 2. Make appropriate referrals based on client case histories and needs. 3. Facilitate one on one and Group-Based Health and Parent Education. 4. Assist clients in completing the Medicaid/CHIP application process and any other benefits for which they are eligible. 5. Provide post-partum doula services to clients based on need. 6. Assist in conducting outreach duties as assigned by the program director. 7. Attend training as assigned by the program director. 8. Comply with safety procedures in accordance with program requirements. 9. Compile, prepare, submit, and maintain accurate records, files, forms, and additional information in accordance with agency policies, licensing, and/or contract requirements. 10. Participate in scheduled meetings and team decisions and operations. 11. Participate in workshops, seminars, education programs, and activities promoting professional development. 12. Obtain and enroll in continuing education/training required for recertification(s). 13. Maintain BCFS HHS professional and ethical standards of conduct outlined in the employee handbook, including demonstrating respect for agency staff, children, and community members and complying with the required dress code, at all times. 14. Provide crisis intervention according to training provided by BCFS HHS when needed to maintain a safe environment.
1. Maintain certificates required for the position including but not limited to First Aid and CPR. 2. Valid Texas Driver’s license with a clear driving record and reliable transportation. 3. Pass a pre-employment drug screen and random drug screens. 4. Provide proof or work eligibility status upon request. 5. Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
