This role offers the opportunity to provide crucial support in clients’ homes and communities, helping them achieve their personal goals and live fulfilling lives. In this position, you will work closely with the Program Coordinator to implement Individual Support Plans, coordinate daily activities, and provide essential services tailored to each client’s unique needs. Your support will empower individuals to thrive within their communities.

Key Responsibilities

  • Work collaboratively in clients’ homes and communities, ensuring personalized care is delivered effectively.
  • Participate in Individual Support Plan meetings and assist with service monitoring visits.
  • Train staff on clients’ medical needs and preferences, ensuring that all team members are informed about medications and compliance.
  • Oversee daily documentation, health tracking, and ensure the implementation of positive behavioral support strategies as needed.
  • Manage household supplies and oversee the maintenance of a safe, clean living environment for clients.
  • Collaborate with the Program Coordinator on follow-ups and compliance with medical and training requirements.

Education, Experience, and/or Credential Qualifications

  • Must be 18 years of age; must have a high school diploma or GED.
  • One year of experience in the field of intellectual/developmental disabilities or successful completion of training in the Missouri Quality Outcomes approved by the Division of DD regional office.

Additional Qualifications

  • Current driver’s license, acceptable driving record, and current auto insurance.
  • Successful completion of a background check including criminal record, driving record, abuse/neglect, and fingerprint check.
  • Completion of New Hire Orientation and all annual training requirements.
