The Social Worker is responsible for the assessment, planning, and evaluation of patient care plans from admission through post-discharge. They utilize case management principles to ensure a seamless and efficient transition of care across the continuum.

Qualifications

Current Licensure in Social Worker (LGSW/LCSW/LICSW) in the District of Columbia. Master’s degree in social work Attendance and Punctuality – Demonstrates the ability to arrive at the work site on time, limits the number of absences, provides adequate notice to supervisor for time off, takes lunch periods and breaks within prescribed times. Mandatory training, testing, and competencies completed on time – Competency Training; Services Excellence Training; Annual Physical. Three years’ experience in acute care or other related healthcare field is preferred. Case Management Certification preferred. Previous experience in case management and/or discharge planning is strongly preferred. Effective oral and written communication and interpersonal skills Knowledge of government and third-party reimbursement systems and contracts. Strong organizational and problem-solving skills. Ability to maintain confidentiality. Ability to adapt to multiple and changing priorities. Requirements

Candidates must hold a Master's degree in social work and current licensure (LGSW/LCSW/LICSW) in the District of Columbia. Three years of acute care experience and Case Management Certification are preferred.
