2nd Shift PCA / Med Tech Department: Resident Care Reports To: Director of Resident Care / Administrator Shift: 2nd Shift
The 2nd Shift PCA / Med Tech is responsible for providing direct care and support to residents during the afternoon and evening hours. This position assists residents with personal care, toileting, meals, showers, activities, evening routines, medication administration when assigned as Med Tech, and maintaining a safe and comfortable environment. The 2nd shift team is responsible for ensuring residents are cared for throughout the evening, properly toileted, assisted with supper, prepared for bed as needed, and left clean, dry, comfortable, and safe before the end of the shift. Essential PCA Responsibilities Assist residents with activities of daily living, including toileting, incontinence care, dressing, grooming, bathing, oral care, transfers, mobility, and meal assistance. Toilet residents every 2 hours and as needed. Change residents promptly when wet or soiled. Complete assigned showers and document completion. Assist residents to and from supper, activities, common areas, and their rooms. Assist residents with evening routines, including changing into pajamas, washing face and hands, oral care, and preparing for bed. Complete assigned lay-downs and ensure residents are positioned safely and comfortably. Answer call lights promptly and respectfully. Complete rounds and safety checks as assigned. Monitor residents for changes in condition, behaviors, pain, weakness, skin concerns, falls, or any unusual symptoms. Report concerns immediately to the Med Tech, Supervisor, Director of Resident Care, or Administrator. Maintain resident rooms by removing trash, soiled briefs, and soiled linens. Keep resident rooms, bathrooms, and common areas neat, clean, and free of clutter. Restock resident supplies as needed, including briefs, wipes, gloves, and hygiene items. Follow resident care plans and service plans. Provide care in a respectful and professional manner. Maintain resident confidentiality and follow HIPAA guidelines. Work as a team and assist coworkers when needed. Essential Med Tech Responsibilities Administer medications as ordered and within scope of training. Complete medication pass accurately and on time. Document medication administration immediately and correctly. Report medication refusals, missed medications, errors, or concerns immediately. Complete treatments, vitals, blood sugars, weights, and other ordered tasks as assigned. Review MAR/TAR and communicate any changes or concerns. Ensure medication cart is kept clean, locked, organized, and stocked. Complete med cart count as required. Process and communicate new orders according to facility procedure. Notify provider, hospice, POA, Director of Resident Care, or Administrator as appropriate. Complete incident reports when required. Communicate resident changes clearly to the next shift. 2nd Shift Specific Duties Receive report from 1st shift before beginning resident care. Review the 24-hour report and any updates from the previous shift. Ensure residents are toileted and ready for supper. Assist residents during supper and monitor meal intake concerns. Return residents safely to rooms, activities, or common areas after supper. Complete assigned evening showers. Complete assigned evening lay-downs. Ensure residents are changed, clean, dry, and comfortable before bedtime. Ensure residents have call lights, water, walkers, wheelchairs, hearing aids, glasses, and personal items within reach as appropriate. Remove soiled linen and trash from resident rooms. Complete assigned room tidies and restock supplies. Assist with laundry and soiled linen as needed. Make sure all residents are safe and comfortable before the end of the shift. Give a complete report to 3rd shift before leaving. Documentation Requirements Document care provided according to facility policy. Document toileting, showers, meals, behaviors, incidents, refusals, and changes in condition as required. Report and document any skin concerns, bruises, redness, wounds, falls, or injuries immediately. Ensure documentation is completed before clocking out. Professional Expectations Arrive to work on time and be ready to begin the shift. Wear proper uniform and name badge. Maintain a positive and professional attitude. Treat residents, families, coworkers, and visitors with respect. Avoid gossip, arguing, or negative communication. Do not use personal cell phones while providing resident care. Stay visible and available on the floor. Follow all facility policies and procedures. Maintain teamwork and assist other halls when needed. Follow direction from the Med Tech, Supervisor, Director of Resident Care, and Administrator. Maintain confidentiality at all times. Physical Requirements This position requires the ability to stand, walk, bend, lift, push, pull, transfer residents, assist with personal care, and respond quickly to resident needs.
