Position Summary

We are seeking a dedicated and compassionate Wound Care Physician Assistant to join our team. This role is built around a collaborative care model, not independent rounding alone. The Physician Assistant will work closely with physicians, nurse practitioners, medical assistants, nursing teams, therapists, dietitians/nutrition support, and other members of the interdisciplinary care team.

This is an excellent opportunity for a Physician Assistant who wants to be part of a welcoming, family-oriented work environment with physician support, teaching, mentorship, clinical development, and scribe assistance.

Responsibilities

Provide wound care consultations and follow-up visits in community and facility-based settings.

Evaluate and manage acute and chronic wounds, including pressure injuries, venous ulcers, diabetic ulcers, arterial wounds, surgical wounds, and traumatic wounds.

Work as part of a collaborative wound care team with physicians, nurse practitioners, physician assistants, medical assistants, nursing staff, therapists, dietitians/nutrition support, and facility leadership.

Help lead and coordinate the interdisciplinary wound care plan with nursing, physical therapy, nutrition, and other support services.

Create individualized wound care treatment plans focused on healing, prevention, and quality outcomes.

Coordinate care with physicians, nursing staff, families, home health agencies, and facility teams.

Participate in wound rounds, education, and clinical quality improvement.

Support wound prevention strategies, risk assessments, and care protocols.

Maintain accurate clinical documentation with support from a medical scribe.

Communicate clearly and professionally with patients, families, and care teams.

Qualifications

Active Physician Assistant license in New York.

Family Medicine, Internal Medicine, Geriatrics, Wound Care, Surgery, Emergency Medicine, or related experience preferred.

Wound care experience preferred, but we are willing to train the right candidate.

Strong clinical judgment and communication skills.

Ability to collaborate with physicians and interdisciplinary clinical teams.

Compassionate, professional, and team-oriented approach to patient care.
