Performing routine and follow-up medical evaluations Managing chronic and acute medical conditions Collaborating with physicians and interdisciplinary care teams Developing individualized treatment plans Reviewing labs, diagnostics, and medications Communicating with patients, families, and nursing staff Helping to prevent unnecessary hospitalizations Focusing on delivering compassionate, patient-centered care Completing documentation with the support of our medical scribes

Active New York State Nurse Practitioner License. DEA Registration (or ability to obtain). Strong clinical assessment and communication skills. Experience in: Family Medicine, Internal Medicine, Geriatrics, Skilled Nursing, Rehabilitation, Long-Term Care, Sub-Acute Care.

Requirements

Candidates must hold an active New York State Nurse Practitioner License and a DEA Registration. Experience in Family Medicine, Internal Medicine, Geriatrics, or Long-Term Care is required.
