  • Range: $24.30 - $25.35 HourlyCase Manager II - Targeted Case Management (Dakota County) People Incorporated is seeking a Case Manager II to join our Targeted Case Management Dakota County team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.

  • Duties: Must be able to have onsite presence within Dakota County, Minnesota Attending bi-annual meetings with clients to monitor services Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications Assessing client needs and ensuring continuity of care. This may include outreach and consultation with other partners and providers. Maintain regular contact with collaborative partners and make referrals to professionals, schools and community agencies, as needed Making referrals and authorizing services based on needs Maintaining records and documenting all coordination activities Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards QualificationsTo perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Requirements

Requires a Bachelor's degree in behavioral science with 2,000 hours of supervised experience or a Master's degree in a related field. Must possess a valid driver's license, auto insurance, and pass all required background screenings.
