Practice for this role

About the role

Provide comprehensive primary care services including preventive, wellness, and acute care for a dedicated patient panel. Manage chronic conditions and coordinate care with interdisciplinary teams to improve long-term patient outcomes.

Advanced practice provider (Nurse practitioner or physician assistant)

Primary Care | Outpatient | Value-Based Care

Location: Westminster, CO

Pay Rate: $130,000 annually

Practice primary care the way it was meant to be practiced.

We are seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our primary care clinic focused on value-based, relationship-driven care. This role is ideal for an APP who believes great primary care is built on time, trust, prevention, and long-term outcomes--not rushed visits and volume-driven metrics.

In our clinic, APPs are trusted primary care providers with their own patient panels, supported by interdisciplinary teams and a care model designed to keep patients healthy, engaged, and out of the hospital.

  • Why this primary care role is different
  • Longitudinal relationships with patients and families
  • Proactive prevention and chronic condition management
  • Care coordination for complex and high-risk patients
  • Quality metrics that reward outcomes--not visit volume
  • Time to listen, educate, and partner with patients
  • What you'll do
  • Provide comprehensive primary care services including preventive, wellness, and acute care visits
  • Diagnose, treat, and manage acute and chronic conditions using evidence-based guidelines
  • Develop and oversee longitudinal care plans focused on prevention and chronic disease optimization
  • Manage medically complex patients and address social determinants of health
  • Prescribe medications and therapies within scope of practice
  • Collaborate with nurses, care coordinators, behavioral health providers, and specialists
  • Use population health data and quality metrics to close care gaps
  • Educate and empower patients to actively participate in their care

What We Offer

  • Competitive base salary
  • Manageable patient volumes
  • Team-based clinical support
  • Clinical autonomy and professional respect
  • Predictable outpatient schedule with no inpatient duties

Compensation & value-Based incentives

We believe high-quality primary care should be supported by compensation that reflects clinical expertise, continuity of care, and patient outcomes--not volume alone.

Compensation for this role includes

  • Competitive base salary aligned with experience, scope of practice, and panel responsibility
  • Value-based incentive opportunities tied to quality measures, preventive care, chronic disease management, and patient engagement
  • Incentives designed to reward thoughtful, proactive care rather than RVU production

For experienced APPs, incentives reflect independent panel management, longitudinal patient outcomes, and contribution to population health goals.

For newer or early-career APPs, the compensation model emphasizes stability, structured onboarding, mentorship, and a gradual ramp-up of panel and incentive expectations.

Our goal is to align compensation with meaningful primary care work--supporting providers who take the time to deliver comprehensive, patient-centered care.

Market context

Colorado advanced-practice roles stay locally competitive

In Colorado, advanced-practice roles often draw steady demand in home-based care and other patient-facing settings, especially for clinicians who can travel locally and work independently in varied home environments. These positions are competitive because employers typically look for an active, unrestricted license, board certification as a Nurse Practitioner or Physician Assistant, and comfort with flexible field-based work. Review the AI-summarized requirements and benefits here to save time, then confirm travel expectations and licensure details before applying.
