Our Mobile Primary Care Team is seeking a dedicated and compassionate Nurse Practitioner/ Physician Assistant or Nurse Practitioner with a heart for senior care and a passion for redefining how older adults and their caregivers experience health and well-being.

As a Nurse Practitioner/ Physician Assistant or Nurse Practitioner, you’ll join a purpose-driven team that: Delivers continuum-based care – integrating primary, psych, palliative, and home-based services.

What Makes this Role Unique Continuum-Based Care (Clinical Excellence): Deliver high-quality care across home, senior living, and community settings, support by a multidisciplinary team in palliative, psych, and neurology.

Dementia Leadership: Build deep expertise in dementia-informed care and caregiver support as part of our expanding geri-psych and cognitive health initiative.

Your responsibilities include: Delivering personalized high-quality care to seniors in their homes, assisted living, hospice and nursing facilities.

Delivering dementia-informed, relationship-based care across the continuum of care.

Conducting comprehensive assessments and developing tailored care plans for older adults.

Managing chronic conditions and performing care with skill and empathy for the patients and their families.

Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.

Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.

Maintaining consistency in documentation, follow-through, and patient communication.

Requirements

Requires an active, unencumbered NP or PA license and 1-2 years of clinical experience, preferably in geriatrics or primary care. Candidates must possess a valid driver's license and reliable transportation.
