As a Home Care Physical Therapist, you'll provide individualized, evidence-based care in patients' homes, including:
Evaluating patients' mobility, strength, balance, and functional abilities Developing and implementing personalized treatment plans to improve movement, function, and independence Providing therapeutic exercises, gait training, balance training, and fall prevention strategies Assessing home environments and recommending safety modifications or assistive devices Educating patients and caregivers on treatment plans, home exercise programs, and injury prevention Collaborating with physicians and interdisciplinary care teams Completing accurate, timely electronic documentation
