About You: DMD or DDS from an accredited U.S. program Active New York Dental License, or ability to obtain a New York License. CPR/BLS Certification Willing to obtain a Real ID for base access Pass a background (Criminal & Malpractice), urinalysis & applicable testing, shots, physical exam and vaccinations.
