DDS or DMD from an accredited U.S. dental program. Completion of an accredited Endodontics residency program. Active, unrestricted state dental license. CPR/BLS Certification. DEA license (if applicable). Ability to pass criminal background check, drug screening, and obtain Real ID for military base access.
