DMD or DDS from an accredited U.S. program Active Dental License (In the state you are applying to) 1–2 years clinical experience CPR/BLS Certification Must possess, or be willing to obtain, a Real ID Drivers License prior to start date. Pass a background (Criminal & Malpractice), urinalysis & applicable testing, shots, physical exam and vaccinations. ________________________________________ If you are made a conditional offer of employment, you will be required to undergo background check (including criminal record check) and drug screening.
