PrimaryOne Health is currently seeking a Hybrid Family Practice & Infectious Disease Certified Nurse Practitioner for our Columbus health centers.

Responsibilities: To develop and implement a standard care arrangement with each supervising physician, updated annually. In compliance with Section 472343 of Ohio Revised Code. Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care. Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral. Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issue. Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatment and/or medications. Maintains legible, accurate, and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations consistent with organization’s regulatory standards. Facilitates evaluation of records by physician(s), peers, and Quality and Standards according to protocols, and receives and implements constructive directives. Analyzes new knowledge gained from conferences, workshops, professional literature, or "hands-on training" and assimilates this knowledge into clinical practice. Ryan White HIV/AIDS Outpatient/Ambulatory Health Services Work as part of a multidisciplinary team that provides comprehensive medical care to patients with special focus on HIV (Human Immunodeficiency Virus) treatment and serving the LGBTQ+ community. Work with various staff to diagnose and manage patients' healthcare. The services will be conducted face-to ­face and/or telehealth visits. Review medical histories and perform physical examinations. Diagnostic testing, including laboratory testing. Treatment and management of physical and behavioral health conditions. Behavioral risk assessment, subsequent counseling, and referral. Preventive care and screening. Prescription and management of medication therapy. Treatment adherence. Education and counseling on health and prevention issues. Referral to and provision of specialty care related to HIV diagnosis. Occasional travel required. Other duties or special projects as assigned. Successful candidates must possess an Ohio State certification as a Nurse Practitioner in the appropriate specialty-pediatric, family, adult, and women’s health, and shall maintain such certification or eligibility during the term of their employment. Requirements

Candidates must possess an Ohio State certification as a Nurse Practitioner in a relevant specialty such as family, pediatric, adult, or women's health. They must be able to maintain this certification throughout their employment.
