About the role

The Registered Dietitian provides comprehensive nutrition assessments, counseling, and care planning for PACE participants. They collaborate with an interdisciplinary team to manage disease-related nutrition concerns and ensure regulatory compliance.

Position located on-site in Grand Junction, CO

Schedule: Monday-Friday, 8:00am-5:00pm

The Registered Dietitian (RD) is responsible for providing comprehensive nutrition assessment, counseling, education, and care planning for participants and participants. The RD collaborates with the interdisciplinary team (IDT) to promote optimal nutritional health, manage disease-related nutrition concerns, and support individualized care goals. This position develops and implements nutrition interventions, monitors outcomes, and ensures compliance with applicable regulatory and professional standards.

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors’ health so they can live safely in their homes. There are 163 PACE programs, serving more than 72,000 participants in 32 states.

What You Bring

Master’s Degree in dietetics, nutrition or related field from an ACEND accredited institution OR Bachelor’s Degree if received prior to 2024.

ACEND-accredited supervised practice hours completed and passed the National Registration Examination for Dietitians required.

Minimum of one (1) year of clinical dietetics experience preferred.

Minimum of one (1) year experience working with older adult population required.

Current registration with the Commission on Dietetics Registration (CDR) required.

CPR Certification maintained throughout employment.

Strong Assessment, counseling, and patient education skills.

Ability to work effectively within an interdisciplinary team environment.

Excellent verbal and written communication skills.

Strong organizational, documentation, and time-management skills.

Proficiency with electronic medical records an standard computer applications.

Requirements

Requires a Bachelor's or Master's degree in dietetics/nutrition, CDR registration, and CPR certification. Candidates must have at least one year of experience working with the older adult population.
