The Role: We have an exciting opportunity for a detail-oriented, candidate who enjoys assisting individuals to reach their full potential. The Case Manager role will provide a rewarding opportunity to work with individuals in skilled nursing facilities to assist them with a transition to the community and ongoing community supports. The Case Manager role requires proven administrative skills, ability to submit documentation in a timely and efficient manner, proficiency with case management tasks. The Rate & Benefits: $22.00 Hourly MAB Community Services offers a comprehensive and flexible benefits package for eligible employees, including but not limited to optional health, dental, and vision insurance, a 403(b) retirement plan, Flexible Spending Accounts (FSA), and a Health Reimbursement Arrangement (HRA) Case Management tasks include any or all of the following: · Housing search and housing applications · Attending housing appointments · Applying for documents, such as an ID or Social Security Card · Coordinating additional assessments, such as PT or OT · Purchase of furniture and essential items · Setting up utilities and purchased items in new homes - Coordinating medical appointments - Assisting with tasks in the community - Coordinating Transportation The Case Manager will work with individuals in the Worcester area and surrounding communities who are transitioning from skilled nursing facilities to living with supports in the community. The Case Manager should be prepared to transport individuals in their own vehicle on occasion and will require some travel to nursing facilities and in the community. This is a hybrid position with occasional travel to nursing facilities, client homes, community locations, and MAB offices. QualificationsThe Requirements:

Must have excellent written and oral communication skills, problem solving skills, and computer literacy.

A minimum of two years experience working with adults with disabilities, mental health, and/or Brain Injuries is required.

A bachelor's degree in a related field is preferred.

Valid driver's license, clean driving record, and reliable transportation required.

Updated CPR and First Aid Certifications required, or ability to become certified immediately upon hire.

Hours are flexible based on program need.
